Online forms
Our appointment and intake forms are hosted by Jotform. Information you submit through those forms is received by us and is also subject to Jotform's privacy policy.
Phone calls, text messages and WhatsApp
If you contact us or give us your phone number, we may call or message you about your inquiry, appointments and services. You can ask us to stop contacting you at any time; for text messages, reply STOP. Message and data rates may apply. Calls related to Medicare enrollment may be recorded as required by Medicare rules.
